Arise Autism Center ("Arise," "we," "us") provides pediatric ABA therapy in Lake Mary, Sanford and Clermont, Florida. This policy explains how we handle information collected through this website, ariseautismcenter.com.
Information you give us
- Family "Get Started" form: parent or guardian name, phone, email, your child's age, insurance company, photos of the front and back of your insurance card, diagnosis status (yes / no / in process), nearest location and preferred therapy setting. We do not ask for your child's name or date of birth on this website.
- Daycare partnership form: your organization's name and contact details.
- Provider relations form (physicians): practice and contact details only. Please do not include patient information on this form — send referrals by fax to (407) 459-1741.
How we use it
We use this information only to respond to you, verify insurance benefits when you authorize it, schedule and coordinate services, and run our partnerships. We do not sell or rent your information, and we do not share it with other companies for their own marketing.
Text messages
If you opt in to text messages, we use your mobile number only to send messages about your inquiry and your child's services. No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text messaging originator opt-in data and consent will not be shared with any third parties. See our Mobile Terms and Conditions.
Where family inquiries go
Family form submissions are sent directly to Arise's secure intake system. They are not stored by our website host. Insurance card photos are uploaded directly from your device to Arise's secure Microsoft 365 storage and are never stored by our website host.
Cookies, analytics and advertising
We use Google Analytics and Google Ads to understand how visitors use our website and to measure whether our ads lead to inquiries. These tools use cookies and similar technologies to collect information such as pages viewed, device and browser type, general location, and how you arrived at our site. When a family form is submitted, Google receives only a notice that a conversion happened — never your name, contact details, insurance, your child's age or anything else you entered. We have turned off Google ad personalization signals and enhanced conversions.
You can opt out of Google Analytics with the Google Analytics Opt-out Browser Add-on (tools.google.com/dlpage/gaoptout), manage Google ad settings at adssettings.google.com, or block cookies in your browser. If your browser sends a Global Privacy Control signal, we do not load these tools.
Health information
This website is for inquiries only. Once your child becomes an Arise client, any health information we hold is protected under HIPAA and handled as described in our Notice of Privacy Practices, which we provide at intake, on request, and online.
Children
This website is intended for parents, guardians, providers and partners. It is not directed to children, and we do not knowingly collect information from children online.
Security
We use reasonable administrative, technical and physical safeguards to protect the information you send us. No method of transmission over the internet is completely secure.
Your choices
You can ask us to access, correct or delete the information you submitted through this website, or to stop contacting you, by calling or emailing us below.

We may update this policy from time to time. The effective date above shows when it was last changed.
